About the Role:
As a Hybrid Pharmacist, you’ll have the unique opportunity to blend operational support with onsite clinical care activities. This dynamic role offers a diverse schedule and the chance to make a meaningful impact on patient care across different levels within the continuum of care.
Under the general direction of the Pharmacy Manager, the Hybrid Pharmacist is responsible for ensuring that all prescriptions are dispensed accurately and safely while providing pharmaceutical patient care ensuring the highest level of therapeutic appropriateness for patients. The Hybrid Pharmacist is responsible for maintaining continuity of care for clientele through collaboration between onsite physicians, nursing staff, and other health providers at long term care, supportive living, and other congregate living communities along with completing clinical pharmacy activities in alignment with federal and provincial standards and regulations. This position is required to provide 24 hour emergency on call support in a rotation with other Pharmacists.
Responsibilities:
- Act as a liaison between personal care homes and pharmacy to ensure optimal Resident care, Pharmacy Services, and Clinical support
- Build and maintain relationships within the community to ensure ongoing collaboration and communication between home/community leadership teams and pharmacy teams to develop/maintain strong partnerships
- Complete medication reviews in currently serviced personal care homes
- Complete all required documentation and billing processes as required by federal and provincial standards and regulations
- Provide education to Residents and staff on various health-related topics
- Conduct education sessions within personal care homes within scope of practice, as applicable (e.g. policy and procedure, web portal, eMAR, influenza protocol, etc.)
- Complete medication audits on-site in personal care homes as required
- Ensure regular on-site visits are made to all assigned clients
- Attend/participate in annual professional conferences and trade shows, when applicable
- Participate in the development and implementation of industry-leading clinical pharmacy service, when applicable
- Be an education, policy and pharmacy initiative resource
- Be available for special clinical team projects as required
- Complete/collaborate in the development of medication audit follow-up improvement plans when applicable
- Work collaboratively with other team members to support applicable homes and communities
- Participate in pharmacy-related transition processes (e.g new client transitions, eMAR transitions, etc.)
- Help with pharmacy operations/drug distribution when required
- Attend and actively participate in clinical team meetings
Qualifications and Skills
Summary of Experience:
- Insured and licensed in good standing with the BC College of Pharmacy Professionals
- Advanced Method Certification required for administering injections
- Previous retail pharmacy experience
- Proven ability to foster strong partnerships with other members of the health care team
- Effective organization, planning, and time management skills
- Demonstrated high level of fluency in both written and spoken English
- Experience with KROLL Pharmacy software required
- Motivated to achieve results that reflect a high standard of quality
- Retail/Community/Assisted Living Pharmacy experience
- Provide evidence of advancing processes for clinical assessment and documentation
